The Rock County Public Health Department and the Health Equity Alliance of Rock County (HEAR) invite you to join Milton’s Community Conversation about health in Rock County. Community Conversations are discussion groups where your input will help choose the focus areas for the next Community Health Improvement Plan. Sharing your thoughts, experiences and insights about health in Rock County can make a difference!

The Milton Community Conversation, for anyone who lives or works in Milton, is scheduled for Wednesday, April 21st from 5:30-7:00PM and will be a virtual conversation over Zoom. During the Community Conversation, a moderator will ask a series of discussion questions and record the feedback from the group. No individual names will be used in the notetaking or reporting of the discussion. Your voice is important because a wide variety of experiences and perspectives will give a more complete picture of health in Rock County. This information will be used to decide which health topics to focus on in the next Community Health Improvement Plan (CHIP). The CHIP will guide the work of the HEAR group for the next 3-5 years.
